The job:

We are located at Yonge Eglinton and work principally in North Toronto and North York.  We provide concierge-level PSW care to families and their loved ones.

If you are a nurse, (RPN) or have a rich background in senior care management, consider working with a team of caregivers dedicated to helping seniors live comfortably in their own homes. Our mission is to support dignity, independence, and quality of life despite age and physical limitations. We aim to help seniors live independently, in their own homes, for as long as possible.  We have a staff of Personal Support Workers (PSW's).  You will be responsible for performing intake assessments and developing Care Plans for new clients.  You will assist in the recruitment of PSW's.  You will supervise a team of caregivers delivering care to seniors.  Scheduling shifts and monitoring client and family satisfaction are key activities.  You will assist in various administrative activities, including payroll and billing cycles.  It is desirable to have experience using case management software (e.g., Alaya Care, Swyftops, etc.) and the Google Suite (Mail, docs, sheets, etc.)

Business growth

The Care Coordinator is a very important part of ComForCare home care, as you form the cornerstone of the new client intake and care supervision.  Being a people person, and providing a high standard of customer service and care are fundamental.

Working with Caregivers

You are comfortable supervising PSW's.  You will get to know their unique skills and make judgements about who is the best match for the needs of the client.   This position may also help to train and instruct caregivers on critical skills that may be necessary when assisting with condition management.

Receiving calls from caregivers and families outside of regular business hours is an occasional part of the job.

This position is ideally suited to those that love working with people and enjoy a regular change of scenery, as opposed to an institutional-style setting.  A Care Coordinator must be highly adaptable, able to work a flexible schedule and evaluate situations quickly and concisely to be a good fit.   You have excellent organizational skills and will work collaboratively with the business development leader and the owner.

Compensation: $55,000.00 per year
